1. Call For Appointment

Please call us at 713-595-9000 first to make an appointment. Alternatively, patients could walk in to our office without an appointment. However, please expect a long waiting time before being seen. When calling for your appointment, we may ask for the following informations: Patient Name, Date of Birth, insurance company, Insurance ID, and Insurance group number. 

2. Obtain Referral (HMO Insurance plans only)

If you have a HMO insurance plan, please make sure to get a referral from your primary care physician, faxed to 713-595-8500. 

3. Fill out new patient forms

Please print and fill out the following forms. Alternatively, you can come to our office and we will hand you the forms to fill out.

• HIPPA / Medical Information Release Form

• Covid-19 screening form

• PHQ9 English, PHQ9 Chinese, or PHQ9 spanish  

4.Visit Our Office

Come visit our office at your scheduled time. Bring your ID, Insurance cards, and the new patient forms you filled out. Sign in at the front desk and we will check you in shortly.
