Please call us at 713-595-9000 first to make an appointment. Alternatively, patients could walk in to our office without an appointment. However, please expect a long wait time before being seen. When calling for your appointment, we may ask for the following informations: Patient Name, Date of Birth, insurance company, Insurance ID, and Insurance group number. 

If you have a HMO insurance plan, please make sure to get a referral from your primary care physician, faxed to 713-595-8500. 

Come visit our office at your scheduled time. Bring your insurance card if there has been any changes. Sign in at the front desk and we will check you in shortly.
